Pre-Kindergarten – 6th Grade Registration for the 2020-2021 School Year

Please have the following requirements when registering Pre-Kindergartners, Kindergartners and New Students:
- Completed 2020-2021 Enrollment Card (available at the school)
- Original Birth Certificate
- Updated 121 Immunization Form
- Social Security Card
- Two updated Proofs of Residency (Driver’s License cannot be used)
Please have the following requirements when registering Returning Students:
- Completed 2020-2021 Enrollment Card (available at the school)
- Two updated Proofs of Residency (Driver’s License cannot be used)

PARENTS/GUARDIANS YOU HAVE TO REGISTER YOUR CHILD FOR THE 2020-2021 SCHOOL YEAR, IF YOUR CHILD ISN’T REGISTERED YOUR CHILD WILL NOT BE ALLOWED TO ATTEND OSYKA ELEMENTARY SCHOOL ONCE SCHOOL BEGIN IN THE FALL FOR THE 2020-2021 SCHOOL YEAR.
